Admission Criteria
Use this guide to decide whether a prospective resident may be a good fit before submitting a referral.
Basic requirements
- Age 21 or older
- Able to live independently in a shared residential setting
- Able to manage personal care and medications
- Medically stable
- Able to follow house rules and maintain safe, respectful behavior
- Payment or an eligible funding arrangement must be verified
- Admission depends on program fit and bed availability
Warm and Grace does not provide
- Needs skilled nursing or hands-on personal care
- Needs medication administration
- Needs detoxification or inpatient behavioral-health treatment
- Needs 24-hour medical supervision
- Presents needs that cannot be safely supported in an independent shared-living setting
Before admission: Each referral receives an individual review, interview, independent-living assessment, funding verification, and availability confirmation.
